Applicant Requirements:

• Taxicab Plateholder Application
• Taxicab Vehicle Replacement Application
• Business Ownership Declaration
• Valid Standard Safety Certificate dated no more than 30 days prior to the date of application.
• Copy of ownership - must be registered in plate holder identification (I.D.) or lessee
• Certificate of Insurance - minimum ($2 million) liability coverage
• Proof that the applicant is not less than 18 years of age
• If a corporation - file Articles of Information, Articles of Amendments or Amalgamation and Annual Return containing list of shareholders.
• If a partnership, the applicant shall provide the identification and addresses of each member of the partnership (see section 4.2, iii).
• If leasing your plate, there is an additional charge for filing such Lease Agreements.
