Birth Death Registration

Births

  • For babies born on or after June 2, 2008, you must register your child (within 30 days of the child's date of birth) directly with Service Ontario, Office of the Registrar General. Service Ontario allows you to register your newborn online.
  • Stillbirths must be registered immediately.
  • The hospital will complete and mail a Notice of Live Birth or Stillbirth (Form 1) to the Office of the Registrar General.
  • After reviewing the information provided, the Office of the Registrar General will mail notification of the registration to the parent(s) or guardian(s) noted on the forms. At this point, the parent(s) or guardian(s) can apply for the Birth Certificate. Birth Certificate applications can be obtained from Service Ontario or from Service Ontario online.

Who may complete the Statement of Live Birth (Form 2)?

The Statement of Live Birth (Form 2) must be completed and filed by:

  1. The mother alone, if the father of the child is not being named on the registration. The child's surname must then be either the present or a former surname of the mother.
  2. The mother and father, if the father is being named on the registration.
  3. The mother, if the father is incapable but is named on the registration, or the father, if the mother is incapable. In this instance, a Statutory Declaration of the Facts (forms are available from Council Services, 350 City Hall Square West Suite 110) must be completed and submitted along with the Statement of Live Birth.
  4. The person standing in place of the child's parents, if the father and mother are incapable. A Statutory Declaration must be submitted stating that both parents are incapable or that the mother is incapable and the father is unknown or unacknowledged by the mother.

Processing Times (birth registrations)

  • Information on processing times is available on the Ministry of Consumer and Business Services website. In addition, clients inquiring about the status of a vital event registration may contact the ORG directly at:
    1-800-461-2156.

Deaths

  • All deaths occurring in the City of Windsor must be registered with City of Windsor Vital Statistics. Required paperwork is then forwarded to the Office of the Registrar General (ORG) for processing.
