Business Licensing By-law 395-2004, Schedule A1 (see By-laws Online)
Applicant Requirements:
- (2) pieces of government-issued identification including one photo I.D. as well as one I.D. demonstrating proof of status in Canada (e.g. birth certificate, Canadian passport, Canadian citizenship card, permanent resident card, work permit, etc.)
- Certificate of Incorporation, if applicable
- Master Business Licence, if applicable
- Business Ownership Declaration for Owner
Owner, Partner or Officer of Corporation - must apply in person:
- Proof of Applicant's right to occupy the premises (deed, lease or agreement)
- Police Records Check (original) - not more than 30 days old
- 2 pieces of I.D. - Government of Canada or Government of Ontario issued (or 1 piece of foreign government I.D. and Canadian government work authorization) with at least one piece showing date of birth and photo (Ontario Health card will not be accepted)
- Board of Health Inspection Certificate - Visit www.wechealthunit.org.
- List of Fees and Services including admission fees and hourly rates (if applicable)
- Copy of Liquor Licence
- List of Operators and Attendants

- 1. Letters of incorporation or Notarial copy of letters
- 2. Annual tax return for most recent taxation year including a list of all shareholders. Are shares held in part, or in whole, by another corporation?
- Partnership - Declaration signed by all partners declaring full I.D. of every partner and address of his/her normal residence, the I.D.'s under which they carry on or intend to carry on business, and business mailing address
- Photo I.D. card issued
Operator - must apply in person:
- Police Records Check (original) - not more than 30 days old
