Copy of Birth/Death Certificate

Any person who applies, pays the prescribed fee, and satisfies the Registrar General as to his/her reason for requiring it, may have a search made for the registration of any Ontario birth, death, marriage or stillbirth in the indexes kept by the Office of the Registrar General.

Copies of birth and death certificates may only be obtained online or by sending an application to The Office of the Registrar General in Thunder Bay.

Application forms for this purpose are available at Service Ontario, Suite 205, 2nd Floor, 400 City Hall Square East and at the Office of the City Clerk, Vital Statistics, Suite 110, 350 City Hall Square West, Windsor.
