Vaccination for Residents - Moderna Information/Consent

​​As announced by the Windsor Essex County Health Unit, the Moderna vaccine is now available to long-term care homes.
As per the direction of the medical officer of health, asymptomatic individuals that are not isolating as a positive case or high risk exposure for COVID-19 are eligible to receive the vaccine. All eligible residents have received both the first and second doses of the Moderna vaccine and remaining residents will be assessed by our medical director on a case by case basis.

Vaccination Information for Designated Caregivers - Pfizer Information/Consent

Since the Pfizer vaccine opportunity was announced, the amount of vaccines anticipated for delivery has frequently changed. Please do not contact Huron Lodge regarding the Pfizer vaccine schedule, as decisions are being made at the federal and provincial levels of government and are outside the control of Huron Lodge or the City of Windsor. We will continue to keep families and caregivers updated with information as it becomes available. If you have delivered proof of vaccination to Huron Lodge, please pick it up when you next attend the home, as only a copy is needed for our records. Designated caregivers should retain the original documents.
