Applicant Requirements:
- Taxicab Plateholder Application
- Taxicab Vehicle Replacement Application
- Business Ownership Declaration
- Valid Standard Safety Certificate dated no more than 30 days prior to the date of application.
- Copy of ownership - must be registered in plate holder identification (I.D.) or lessee
- Certificate of Insurance - minimum ($2 million) liability coverage
- Proof that the applicant is not less than 18 years of age
- If a corporation - file Articles of Information, Articles of Amendments or Amalgamation and Annual Return containing list of shareholders.
- If a partnership, the applicant shall provide the identification and addresses of each member of the partnership (see section 4.2, iii).
- If leasing your plate, there is an additional charge for filing such Lease Agreements.
